CONTROVERSIES IN OPHTHALMOLOGY Microbial keratitis: what’s the preferred initial therapy?

چکیده
A29 year old woman, who was an intermittent contact lens wearer, is referred from her local ophthalmologist with a diagnosis of microbial keratitis. There is a large central corneal ulcer with stromal white cell recruitment and low grade anterior chamber (AC) activity. The patient has no other eye problems and is in good health. Would you perform a corneal scraping? Which antibiotics would you use to begin empirical treatment for her microbial keratitis?

متن کاملHost, microbial, and pharmacological factors affecting the outcome of suppurative keratitis.
A review of 87 cases of microbial keratitis in South Australia was made to determine the factors which influence the outcome of the disease. The preceding pathology and the extent of ulceration at presentation were found to be significant, while the presence of hypopyon was less important. Infection with Pseudomonas aeruginosa was most likely to result in a poor outcome. Most patients had a his...
متن کاملStrategies for the management of microbial keratitis.
Microbial keratitis results from a complex interaction between a protean array of pathogens and a diversity of host responses. The variability of these factors has hampered the development of simple management guidelines that will ensure an optimal outcome for most cases; a consequence of this has been the use of an extensive range of culture media in initial investigation, toxic levels of anti...
متن کاملKOH mount as an aid in the management of infectious keratitis at secondary eye care centre.
AIMS To report the clinical outcome of infectious keratitis managed after doing 10% KOH mount of corneal smears and reporting done by an ophthalmologist in the secondary eye care centre in South India. METHODS 103 consecutive cases of microbial keratitis were studied. Inclusion criteria were presence of corneal infiltrate on slit lamp biomicroscopy. An ophthalmologist carried out microbiologi...
متن کاملMixed infection (Pseudomonas and coagulase negative staphylococci) microbial keratitis associated with extended wear silicone hydrogel contact lens.
Contact lens induced ulcerative keratitis is a serious complication which can be devastating for the patient if treatment is delayed. Extended wear is the commonest cause of microbial keratitis in contact lens wear. New extended wear silicone hydrogel contact lenses have higher oxygen transmissibility so that they can be worn continuously for 30 days. They can also be used as bandage contact le...
